We request that you provide prior medical or vaccine records from your previous veterinarian to give us a little more history on your pet. This can help us provide the best care possible for your pet.
You may obtain copies of your pet’s records from previous veterinarians by requesting hard copies for pick up, mail, fax or email. Please be sure to have all documents available for your appointment. For your convenience, you may also request your previous veterinarian to send your pets medical records directly to our facility via fax at
